If you are a new patient to our office, the attached file contains our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Being well prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It will also help relieve any unnecessary anxiety you may be feeling. Fill out any new patient forms, which can be found on this site, beforehand, educate yourself on your symptoms or on any issues that you may be having, have your questions written out prior to your appointment and familiarize yourself with the variety of services that we offer. What to Expect:
Your first visit to Asay Dentistry establishes a vital foundation for us to build upon. During your first visit with us, we will obtain important background information, such as your medical and dental history. Generally, during your first visit, we will also allot more time for you to get to know us and ask any questions you may have. Your initial appointment will also include a comprehensive oral evaluation, screening, periodontal records and xrays.

At Asay Dentistry, we stive to offer high quality, affordable dental services to care for our patients while also helping the costs fit into their budgets. The cost of your dental treatment will vary on your individual needs and the treatment plan that you choose.
Our staff will discuss with you your treatment options, the cost of treatment, and any insurance or payment options available to you.
To aid in providing the best care at the best financial option for you, we accept most dental insurance plans and all major credit cards and will gladly submit any insurance claims on your behalf.
